OF HEAVYWEIGHT VEHICLE

This certification is required when registering trucks that weigh at least 44,801 lbs. This requirement
applies to vehicles registered in the Agricultural (AGR), Commercial (COM), Commercial Household
Carrier (CHC), Tractor (TRC) and Apportioned (IRP) classes.
I certify that the following information is correct with respect to the construction of the vehicle being
registered:
Vehicle Identification Number
Number of axles
Wheelbase*
_____________Feet ___________ Inches
*Wheelbase is the distance from the center of the frontmost axle to the center of the
rearmost axle. Wheelbase is not required for 2-axle trucks.
