1. Title and Signature Page
The first page or cover page of each SOP should contain the following information: a
title that clearly identifies the activity or procedure, SOP identification (ID) number,
effective date, the name of the applicable agency, division, and/or branch to which this
SOP applies, and the signatures and signature dates of those individuals who prepared
and approved the SOP. Electronic signatures are acceptable for SOPs maintained solely
on a computerized database; otherwise, scanned images of printed signatures must be
maintained.
The document control table located in the header of the document includes:
The document ID which will be assigned by the quality assurance coordinator for
the branch, or the quality assurance officer for the Division. Contact those
individuals for the document ID identifier.
The revision # is the iteration of the SOP that is the most recent. Using 0.0
indicates the first iteration, 1.0, 2.0 used consecutively thereafter.
The effective date indicates the date that the SOP has been approved and is now
usable.
The page numbers indicate the total number of pages, outlined in X of Y format.
2. Document Revision Page
The revision control page should clearly outline what revisions have occurred for the
document, the date of revisions, on what page the revisions occur. The most recent
revision should be listed first.
3. Table of Contents (TOC)
A Table of Contents is needed for quick reference, especially if the SOP is lengthy, for
locating information and to denote changes or revisions made only to certain sections of
an SOP. A TOC should include a list of tables and a list of figures. If using a pdf format
or creating an electronic TOC in Word©, bookmarks can be created and used more
efficiently.
4. Scope and Applicability
State who and what the SOP covers. Is it for a section, branch, division, etc.? Be specific.
